Clinical Quality Improvement Specialist, Remote

The Clinical Quality Improvement Specialist is responsible for leading clinical innovation and transformation initiatives in collaboration with Aledade market leaders and clinical leads within primary care practices. This role focuses on optimizing clinical and quality outcomes while reducing unnecessary costs for defined patient populations, such as individuals with uncontrolled hypertension or diabetes.

This position requires a strong commitment to Aledade’s mission of advancing clinician-led accountable care organizations (ACOs) to improve healthcare quality, enhance the patient experience, reduce costs, and promote health equity.

Primary Duties:

  • Lead clinical innovation projects and initiatives designed  to support partner practices in achieving success in value-based care for targeted patient populations.
  • Utilize clinical process measures and outcomes data to develop and execute action plans focused on  population health improvement and reducing health disparities. 
  • Mentor, coach, and collaborate with key practice team members to effectively implement  clinical innovation and transformation strategies. 
  • Provide education and support to both practice and internal team members.
  • Champion the development, maintenance, and optimization of resources, tools, and educational materials to support quality improvement in partner practices.

    Minimum Qualifications:

  • Bachelor's degree in nursing with the degree from a nursing accredited college or university
  • Active Registered Nurse (RN) license with 3+ years of registered nursing experience
  • ,

    Preferred Qualifications:

  • 2+ years working in value-based care
  • Experience with quality improvement
  • Experience in a Community Health Center or Primary Care Practice setting 
  • Strong understanding of value-based care principles, particularly as they relate to population health improvement.
  • A solution-oriented individual who can assist in creating tactical clinical innovation then translate those solutions to care teams (including executives, clinicians, practice managers, clinic and system owners, nurses, and all ancillary care team members) to execute towards delivering population health results.
  • Excellent communicator who can articulate the impact of clinical innovation and transformation initiatives to Aledade ACO member practices and their key staff (e.g., nurses/other clinicians, care managers, etc.), and internally within the company. 
  • Ability to use insights from clinical, quality, and cost data to inform clinical innovation and transformation and proactively address opportunities for improvement.
  • Ability to work collaboratively across Aledade clinical and non-clinical teams to gain buy-in and implement key clinical innovation initiatives.
  • Ability to work with diverse populations with a passion for health equity and eliminating health disparities.
  • Skilled at coaching practice staff and internal teams using an evidence-based, data-driven approach to effect the adoption of clinical innovation and transformation.
  • Collaborate effectively with clinicians to pinpoint and recommend improvements to clinical workflows.
